The Opportunity: 

Abby Care is seeking a skilled, compassionate Registered Nurse for a dual-function role combining hands-on patient case management with clinical education and caregiver training. Reporting to the Director of Nursing, you will manage a diverse patient caseload, coordinate comprehensive care, and serve as a Clinical Instructor to train and prepare the next generation of Home Health Aides (HHAs). In this position, you will collaborate closely with clinical teams, primary care physicians (PCPs), family caregivers, and HHA students to deliver quality clinical care while building the workforce that supports it.

Data-driven and quality-focused, you have a passion for maintaining rigorous clinical documentation standards, reducing patient readmissions, and empowering students and family caregivers through clear communication and mentorship. 

This is a Full-Time Hybrid opportunity based in State College, PA. 

What you’ll work on: 

  • Manage patient care: Coordinate and deliver comprehensive care for a diverse patient caseload while developing and implementing individualized care plans. 
  • Collaborate with care teams: Work closely with PCPs, family caregivers, and internal clinical teams to ensure seamless care delivery and alignment on treatment goals.
  • Instruct and train HHAs: Design and deliver skill training classes and clinical education modules for HHA students, enhancing their medical knowledge and practical abilities.
  • Provide clinical supervision: Support and oversee HHA students and caregivers during hands-on training at long-term care facilities and home settings to ensure adherence to clinical standards. 
  • Maintain electronic documentation: Complete accurate electronic charting within required timelines, adhering strictly to regulatory and Abby Care standards (minimum 24-hour turnaround). 
  • Educate patients and families: Guide and empower patients and their family caregivers on ongoing health needs, disease management, and treatment plans.
  • Escalate clinical risks: Promptly identify and escalate complex patient issues or clinical concerns to Clinical Leads. 
  • Participate in coverage rotation: Support patient needs by participating in the team on-call rotation (approximately 1 weekend per month) and holiday coverage.
  • Drive continuous improvement: Monitor key quality metrics, including patient readmission rates and caregiver satisfaction, to continuously elevate clinical performance. 

What success looks like:

In 12 months, you will have successfully managed a well-coordinated patient caseload with low readmission rates and high caregiver satisfaction, while graduating confident, highly skilled HHA cohorts through engaging training programs. You will be recognized as a reliable clinical resource who consistently meets documentation timelines and collaborates seamlessly across multidisciplinary teams. 

What you’ll have: 

Required Qualifications: 

  • Current, valid unencumbered RN license in Pennsylvania or a Compact/Multi-State license. 
  • Associate's or Baccalaureate degree in Nursing (ADN or BSN). 
  • 2+ years of clinical RN experience. 
  • 1+ year of home health experience. 
  • Valid driver's license, reliable personal transportation, and current auto insurance.
  • Must reside or be willing to permanently relocate to the State College, PA area.
  • Ability to work flexible hours, including participation in an on-call rotation (approx. 1 weekend/month) and holiday coverage. 
  • Excellent communication, interpersonal, and instructional skills. 

Preferred Qualifications: 

  • Experience in curriculum development, staff training, or clinical instruction.
  • Experience with real-time electronic health record (EHR) charting and clinical documentation standards.

Pay range: $80,000-$86,000/year
