Position Summary
Safer Home Care Inc. is seeking compassionate, dependable, and professional Direct Care Workers (DCWs) to provide non-skilled home and community-based services to participants enrolled in Pennsylvania Community HealthChoices (CHC) and waiver programs in Erie and Crawford County. Direct Care Workers assist participants with activities of daily living (ADLs), companionship, safety monitoring, and community independence while maintaining participant dignity, rights, and person-centered care.
This position supports individuals in their homes and community settings in accordance with Pennsylvania Office of Long-Term Living (OLTL), Community HealthChoices (CHC), and Pennsylvania Department of Health (DOH) guidelines. (Pennsylvania Government)
Essential Job Duties
• Assist participants with activities of daily living (ADLs), including:
Bathing
Grooming
Dressing
Toileting
Mobility/transfers
Ambulation assistance
Meal preparation and feeding assistance
• Assist with instrumental activities of daily living (IADLs), including:
Light housekeeping
Laundry
Grocery shopping
Medication reminders
Transportation/errands
Appointment accompaniment
• Provide companionship and emotional support
• Observe and report changes in participant condition, environment, or safety concerns to the office immediately
• Maintain accurate daily documentation and EVV (Electronic Visit Verification) compliance
• Follow participant care plans and agency policies at all times
• Maintain participant confidentiality and HIPAA compliance
• Report suspected abuse, neglect, exploitation, or critical incidents per OLTL and agency policy requirements (Pennsylvania Government)
• Maintain a clean, safe, and respectful care environment
• Arrive to scheduled shifts on time and maintain reliable attendance
Minimum Qualifications
• Must be at least 18 years old
• High school diploma or GED preferred
• Valid driver’s license, reliable transportation, and current auto insurance preferred
• Ability to pass:
Pennsylvania State Police Clearance (Act 34)
Child Abuse Clearance (Act 151)
FBI Fingerprinting if applicable (Act 114)
OIG exclusion screening
Reference verification
• Ability to complete agency orientation, competency testing, and required annual in-services/training
• Ability to communicate effectively with participants, families, and office staff
• Ability to safely lift, transfer, and assist participants as required
Preferred Qualifications
• Previous caregiver, PCA, HHA, CNA, or home care experience preferred
• Experience working with elderly or disabled individuals
• CPR/First Aid certification preferred

• Understanding of infection control and safety procedures
• Ability to follow care plans and report concerns appropriately
• Ability to use EVV systems and mobile applications

Compliance Requirements
Employees must comply with all applicable Pennsylvania Department of Health (DOH), Office of Long-Term Living (OLTL), Community HealthChoices (CHC), Medicaid waiver, and agency requirements including training, documentation, incident reporting, EVV usage, confidentiality, participant rights, and annual competency requirements.
