Position Summary / Position Overview
This position is a front-line, client facing growth driver; responsible for increasing the agency’s census through referral development, community outreach, and high performing intake execution. This role blends recruiting, sales, true relationship building and waivered based intake operations.
Applicants should have knowledge of the waiver enrollment process, understanding of Area on Aging, Maximus (PA enrollment broker), and support coordinators, managed care organizations (MCOs) and Community Health Choices (CHC).
Success in this role is measured by clear growth metrics (not just activity), compliance, service quality, referral quality and measurable impact on our community.
This role is front facing, and results driven. Success is evaluated by:
· Monthly family caregiver and participant growth’
· Number of active referral sources
· Lead to intake conversion
· Conversion time
· Participant satisfaction and retention
· Consistent outreach and activity reporting
Duties and Responsibilities
· Build and maintain strong referral relationships with:
- Ø Local Area Agencies on Aging (AAA)
- Ø Support Coordinators
- Ø Hospital discharge planners, case managers and social workers
- Ø Senior Centers and community organizations
· Conduct face to face outreach, networking and presentations in the community
· Identify and pursue new referral opportunities’
· Serve as the primary community ambassador for the agency
· Track referral leads and follow through to conversion
· Conduct participant assessments and complete Medicaid waiver intakes
· Recruit, onboard and enroll family caregivers
· Ensure all paperwork is accurate, complete, compliant and submitted in a timely manner
· Maintain organized compliant case notes
· Complete follow-up visits to ensure satisfaction, continuity of care and retention
Preferred Experience

· Health care sales, community outreach, intake coordination or case management

· Medicaid Waiver Programs, OLTL and CHC experience a strong plus

· Referral development or census growth experience
Requirements

· Clear cut understanding of professionalism and true customer service

· Ability to work independently as well as with a strong team

· Valid Driver’s License and reliable transportation

· Ability to travel locally for outreach and intake appointments

· Flexibility in schedule to attend community events when needed.
