We provide compassionate, non-medical home care services to families across Philadelphia, Bucks, Chester, Delaware, and Montgomery counties. We’re looking for a motivated Part-Time Home Care Community Liaison to expand our network, build referral relationships, and connect families with the care they need.
Position Summary
As a Community Liaison, you will develop and maintain relationships with hospitals, rehabilitation centers, senior communities, and local organizations. This healthcare sales and community outreach role is perfect for someone who enjoys networking, building partnerships, and educating the community about home care services. This is a part-time position with flexible hours, offering the potential to transition into a full-time role based on performance and growth.
Responsibilities
- Establish and maintain relationships with referral sources and community organizations.
- Conduct in-services and presentations to educate referral sources and community partners about home care services.
- Represent the company at health fairs, community events, networking meetings, and senior-focused programs.
- Track leads and referrals, follow up with prospects, and coordinate initial intake visits to ensure smooth onboarding of new clients.
- Monitor referral outcomes and maintain activity reports and marketing performance data.
- Stay up to date with industry trends and local community needs.
- Act as a referral specialist, helping connect families to the right services at the right time.
Qualifications
- 2+ years of experience in healthcare marketing, home care, senior living, or community outreach..
- Strong communication and interpersonal skills; comfortable speaking with professionals and families alike.
- Ability to travel throughout Philadelphia and surrounding counties.
- Self-starter with the ability to work independently and manage time effectively.
- Bachelor's degree in Marketing, Healthcare Administration, Social Work, or related field preferred.
- Valid driver’s license and reliable transportation required.

Commission Structure for Referral Bonuses:
- $200 for clients who continue services for 4+ weeks (20+ hrs/week)
- $300 for long-term clients active for 8+ weeks (20+ hrs/week)
