What You’ll Do:
As our new Occupational Therapist, you’ll be at the heart of our care team, delivering personalized therapy services that empower patients to live more comfortably and confidently. Your responsibilities will include:

• Providing direct therapy treatments in patients’ homes

• Assessing home environments and developing personalized care plans

• Collaborating with physicians and other healthcare professionals

• Monitoring and reporting patient progress and adapting plans as needed

• Identifying additional care needs and coordinating with home health services

• Supervising OT assistants in accordance with policy and regulations

• Supporting patients and families through discharge planning and beyond

What You’ll Bring:
✅ Degree from an accredited OT program (Bachelor’s or Master’s)
✅ Active OT license in Pennsylvania
✅ Minimum 2 years of OT experience (home health experience is a plus!)
✅ Current CPR certification
✅ Valid driver’s license & reliable transportation
