Intake/Onboarding Specialist (Bilingual - Spanish)

Experience in Onboarding & Recruitment is highly desirable. Must be willing to work in a fast-paced staffing environment. Must be Bilingual (English/Spanish).

Pay: Up to $22/hr. + Incentive Program (additional compensation is provided for meeting individual & team metrics determined by your department manager).

PRIMARY RESPONSIBILITIES -

Onboarding:

1. Responsible for onboarding the direct care workers at Aging with Comfort which entails completing the interview and initial personnel screening of the direct care worker including but not limited to: reference checks, obtaining proof of residency, criminal background checks, initiating the exclusion checks, updating results in the file, and setting up the TB screening process.

2. Creates the direct care worker’s ID badge.

3. Completes the tracking of all data on the personnel file checklist and creates the direct care worker’s personnel file and then uploads all personnel data into the HHAeXchange HR system so that all data can be tracked for compliance purposes.

4. Completes the direct care worker’s orientation, competency training and skills testing and documents on the agency required forms before placing in the personnel file and logging the information into the HHAeXchange HR system.

5. Once the direct care worker’s personnel file has been completed, forwards the file to the team lead for final review.

6. Introduces the direct care worker to the Care Manager so that services may begin being provided to the consumer.

7. Sets up process to be followed for provisional hiring of the direct care worker, if necessary

Intake:

1. Enters new consumer referral into Aging with Comfort CRM software.

2. Obtains referral from the consumer’s MCO or if Medicaid covered only, from Welfare Office for Medicaid coverage.

3. Enters intake data regarding consumer services and needs into the agency’s electronic medical record (EMR), HHAeXchange and obtains any information/data available regarding location of care, care/services needed by the consumer, emergency contacts, etc. This data may be gathered from sources which may include but not be limited to any of the following:

a. Referral sources such as a community agency, hospital, physician’s office, Area Agency on Agency, skilled nursing facility, assisted living facility, personal care home, etc.;

b. Information gathered by the Aging with Comfort Community Outreach worker who obtained the referral;

c. Transfer information from another home care agency, if consumer is being transferred to Aging with Comfort; or

d. Information shared by the consumer, family member, caregivers or significant others.

4. Upon completion of all intake data, the Intake Specialist introduces the consumer to the Care Coordinator who will then assign the direct care worker (caregiver) and oversee the care and case.

HUMAN RELATIONS REQUIREMENTS -

1. Must maintain a professional attitude and demeanor and be able to communicate effectively and relate courteously and cooperatively with consumers, caregivers, colleagues, supervisor, co-workers and all others.

2. Must be able to demonstrate optimism, enthusiasm and willingness to work constructively with other home care team members and reflect concern for the well-being of clients and others.

PHYSICAL REQUIREMENTS

1. Able to operate basic office equipment (copier, fax, computer, telephone)

2. Ability to frequently stand, sit, bend, lift, carry, walk to perform the essential responsibilities of the position.

3. Visual/hearing ability must be sufficient to comprehend written and verbal communication and include excellent listening skills.

QUALIFICATIONS -

1. Education

a. High School Diploma or GED required.

b. Associate degree in human resources, sociology or equivalent preferred

2. Experience

a. Minimum of two years’ experience in home care, preferably in client intake/relations, human resources, or orienting and training staff.

b. Must have excellent organizational, communication (written and oral), and interpersonal skills, including being able to demonstrate flexibility, adaptability, tolerance, patience, and tact.

c. The candidate must have experience of interacting effectively with direct care workers and be able to interview, train and teach.

d. Must also have the ability to maintain confidentiality, work independently, be a team player, and be detail oriented.

3. Other Requirements

a. Possesses a valid Pennsylvania driver’s license with reliable transportation insured in accordance with Pennsylvania state laws and regulations.

b. Has satisfactory references from employers and/or professional peers.

c. Possesses satisfactory criminal background check.

Job Type: Full-time

Pay: $18.00 - $22.00 per hour
