Hospice RN Case Manager

Vitality Hospice & Palliative Care is seeking an exceptional Registered Nurse to join our team! Position is Full Time, Monday through Friday 8am-5pm with occasional night/weekend needs.

Responsibilities:

As a Registered Nurse Case Manager (RNCM), you will have the following responsibilities:

• Provide professional nursing care, complete admission, initial comprehensive, and ongoing assessments pf patients.
• Initiate, develop and update individual appropriate plan of care according to an individual patient's needs.
• Working collaboratively with the hospice physician and Interdisciplinary Team to carry out appropriate interventions.
• Managing and supervising the combined efforts of home health aides, social workers, and chaplains.
• Educate patients and caregivers as appropriate.
• Upholding high standards of compliance regarding documentation and patient care requirements.
• Helping the agency to continuously increase the quality of care.

Qualifications:

• Current PA Registered Nurse (RN) License
• Experience in Nursing
• Willing to travel to the assigned areas
• Must have a valid drivers’ license with reliable transportation and liability insurance
• Strong communication and interpersonal skills
