Hospice Admissions Nurse

Vitality Hospice & Palliative Care is seeking an exceptional Registered Nurse to join our team! Responsibilities:

• Conduct thorough assessments of patients and their families to ensure proper care planning.
• Provide direct patient care, including monitoring, administering medications, and performing treatments.
• Collaborate with the healthcare team to develop individualized care plans.
• Facilitate communication between patients, families, and healthcare providers.
• Educate patients and families about disease management, treatment, and self-care techniques.

Qualifications:

• Valid nursing license and relevant certifications in hospice care
• 1+ years of prior skilled nursing experience preferred
• Willing to travel to the assigned areas
• Strong communication and interpersonal skills
• Valid driver’s license, auto liability coverage, and reliable transportation

Schedule & Benefits:

• Work Schedule: Tuesday to Friday, 10:00 AM to 8:30 PM.
