Job Summary: We are seeking a caring, highly skilled Registered Nurse (RN) to deliver home health care services to pediatric patients. As a part of our dedicated team, you will work with pediatric patients, providing clinical nursing care in a one-on-one, home-based setting. You will play a vital role in coordinating care, educating pediatric patients and families, and ensuring the best possible outcomes for each patient.

Key Responsibilities

  • RN will perform home visit to pediatric patients and coordinate case management activities across a multi-disciplinary health care team to ensure appropriate, quality patient care.
  • Establish patients’ plans of care (485) in collaboration with physicians, patients, and utilization management (UM) professionals to meet physicians’ orders and patients’ needs.
  • Patient Care: Conduct comprehensive assessments, monitor pediatric patient health status, and provide skilled pediatric nursing care in the home. Complete all required documentation including visit assessments, OASIS, 485 and verbal orders accurately and promptly on a laptop or in written documentation while in the patient's home and according to all applicable regulatory standards.
  • Attend compliance training and adhere to the organization’s standards of conduct, policies, and procedures in addition to all local, state, and federal regulatory guidelines.
  • Care Planning: Develop and implement individualized care plans based on patient needs and physician orders, regularly reassessing and adjusting plans to ensure optimal care.
  • Collaboration: Work closely with physicians, social workers, and other healthcare professionals to coordinate patient care and address any issues.
  • Education & Support: Educate patients and their families on disease prevention, medication adherence, and self-care techniques to improve health outcomes.
  • Documentation & Compliance: Maintain accurate, timely, and comprehensive electronic medical records (EMRs) in accordance with agency and regulatory standards.
  • Patient Advocacy: Ensure that patients receive the highest quality of care, promoting their dignity, independence, and well-being.
  • Safety & Quality Assurance: Monitor patient safety, prevent complications, and provide interventions as needed. Ensure that all care is delivered in compliance with agency policies, industry standards, and state/federal regulations.
  • Licensure: Current, valid RN license in state of Pennsylvania.
  • Experience: Minimum of 1-2 years of clinical nursing experience in health care or related settings.
  • Certifications: BLS (Basic Life Support) required.
  • Skills: Strong clinical assessment skills, critical thinking abilities, and proficiency with electronic health records (EHR) systems.
  • Communication: Excellent interpersonal and communication skills, with the ability to educate patients, collaborate with interdisciplinary teams, and support families.
  • Independence: Ability to work autonomously and manage a caseload efficiently while adhering to care plans and safety protocols.
  • Transportation: Must have a valid driver’s license, reliable transportation, and auto insurance to travel to patient homes.

Preferred Qualifications:

  • Education: An Associate Degree in Nursing (ADN).
  • Specializations: Pediatrics nursing experience preferred.
  • Regulatory Knowledge: Familiarity with home health regulations, Medicare/Medicaid guidelines, and accreditation standards (e.g., ACHC, DOH) is a plus.
